Care inspectors have praised the work of the Vale Community Resource Service based at Barry Hospital.
A new report found it facility helped residents to regain their confidence and independence following illness.
The service offers short-term reablement care for patients after hospital stays or after they are referred by community services to regain daily living skills.
Care Inspectorate Wales noted: "People are extremely complimentary about the service they receive and highly praise staff who provide the service."
"This includes specialist reablement practitioners which means people’s recovery and reablement is maximised. People receive day-to-day care and support from skilled and dedicated reablement support workers (RSWs)."
"We rate well-being as excellent because of the overwhelming, positive impact the service is contributing to people’s outcomes, and their emotional and physical well-being. The culture of the organisation is united, with reablement for people at the heart of the service."
Vale of Glamorgan Council said it welcomed the inspectors' findings.
Cllr Eddie Williams, cabinet member for social care and health, said: "This report is a testament to the dedication and compassion of the Vale Community Resource Service team. Their work enables people to return to independence with confidence, dignity, and the right support in place."
"We are incredibly proud of the recognition they have received and remain committed to ensuring that our services put people’s wellbeing at the heart of everything we do."
